Complete our universal Application for Service for any Rising Treetops at Oakhurst services. You can request a paper application by calling 732-531-0215 or 212-533-4020 or by emailing services@risingtreetops.org, or you can download and print a copy of our universal Application for Service. Applications are received throughout the year. Confirmation for a respite or camp session can only be made after: 1) receiving the Application and supporting documents, 2) having an intake interview (in person or by zoom/video chat), 3) after an applicant’s appropriateness to be served in Rising Treetops’ service and staffing model is determined, as well as 4) space availability.
